I built Seniority Solutions because I repeatedly saw seniors and families facing delays in care, difficulty accessing services, and challenges navigating an often-complex healthcare system.
I wanted to create a service that provides personalized guidance, advocacy, and support to help families move forward with confidence. I'm Lissette Fernandez, social worker, certified case manager, and founder of Seniority Solutions. I've spent more than 20 years working inside hospitals, doctor's offices and care facilities.
What I saw, over and over, was the same thing: seniors and families navigating one of the hardest seasons of their lives with no real guidance. Families were often left trying to navigate complex care decisions, coordinate services, and manage important information during one of the most challenging times in their lives. Many became the primary advocate and coordinator for their loved one, often without the guidance and support they needed.
Care transitions often left families feeling overwhelmed and uncertain about the next steps. At the same time, valuable benefits and resources including VA programs, Medicaid waivers, and other community support services were frequently overlooked simply because families did not know they existed or how to access them.
That's why I founded Seniority Solutions. I created this business to provide the kind of guidance and support I often wished families had someone who stays with them through the process, advocates for their loved one, coordinates the many moving pieces, and helps ensure no opportunity, resource, or important detail is overlooked.
